'co located POCs unavailable 0B f=0/0' usually hints to a stream that is somehow broken.
Since this only appeared once it might just be a small glitch at the beginning.
Try remuxing the file with mkvtoolnix before handling it with Hybrid. mkvtoolnix might be able to remux the file.
