Changing options to Slow Preset sometimes changes to Medium

That is correct.
Hybrid minimizes the command line depending on the settings you use, if you settings have more in common with the medium than the slow preset Hybrid will use that preset in the command line to keep the command line short.
If you don't like that disable 'minimize command line' under Misc, then Hybrid will not use '--preset XX' or '--tune XX', but instead only show the individual options.
